A few useful checks.
Why is recording unavailable?
Open Recording settings and check Permissions & speech. Confirm the language model is ready and another recording or import is not still processing.
Why is playback missing?
Playback needs retained source media. Changing Keep audio for playback affects new recordings and cannot recreate audio that was never saved.
Why did an action scan find nothing?
No proposals means that scan found none, not that no commitments exist. Check partial-review notices, use Quick scan or save an action from a passage.
